ABSTRAK (i-viii + 75 halaman + 9 tabel + 2 gambar) Asfiksia neonatorum adalah suatu kondisi dimana bayi tidak segera bernapas dan tidak bernapas secara teratur setelah lahir, kondisi ini disebabkan oleh hipoksia janin didalam rahim yang disebabkan oleh faktor-faktor yang muncul selama kehamilan, persalinan atau segera setelah lahir. Tujuan dari penelitian ini menerapkan asuhan keperawatan pada bayi Ny. R dengan diagnosa pola nafas tidak efektif dalam tinjauan teori Adaptasi Roy di ruangan Nicu RSUD Anutapura Palu Provinsi Sulawesi Tengah. Metode penyusunan Karya Ilmiah Akhir ini menggunakan metode deskriptif dalam bentuk studi kasus dengan pendekatan proses keperawatan meliputi : pengkajian, diagnosa keperawatan, perencanaan keperawatan, implementasi, dan evaluasi selama 3 hari. Hasil penelitian bayi Ny.R mengalami sesak nafas, tampak retraksi dinding dada, fase ekspirasi lebih panjang dari inspirasi dengan frekuensi nafas 65x/menit, nadi 130x/menit, SPO2 94%, dan bayi terpasang O2 nasal kanul 0.5 liter/menit. Diagnosa keperawatan prioritas yaitu pola nafas tidak efektif berhubungan dengan hambatan upaya napas. Intervensi yang dilakukan yaitu dengan mengatur posisi pronasi pasien dengan kolaborasi pemberian terapi untuk membantu membebaskan jalan nafas. Hasil menunjukan pola nafas tidak efektif tercapai.
